Access Point 4 Housing (AP4H): As part of the strategic plan to end homelessness in Pierce County and design a “best practice” model to prevent homelessness, Associated Ministries had contracted with Pierce County to provide a centralized intake center for homeless individuals and families and those in imminent risk of becoming homeless.
Navigation Services: As part of AP4H coordinated entry, a Housing Specialist will work one-on-one with individuals to enable stability. Services will include, but are not limited to, referrals to housing programs or independent housing, assistance/referrals in securing benefits, referrals to other prevention providers or funding sources, mental health and substance abuse services, and referrals for job preparation or employment services.
AP4H Eligibility Criteria: This program offers a central point of entry for households experiencing homelessness (or will be homeless within 72 hours). Households receive resources to secure housing and other needed services.
Office Hours for AP4H: Mon.-Fri. 9am-5pm (some evening appointments available), closed for lunch 12-1pm.
